APPOINTMENTS

When you call, you are given the first available appointment.  Please call our office at 478-741-1740, if you are unable to keep your appointment. Please bring photo ID, insurance cards, list of medications, all eye drops you are using and method of payment.

 OFFICE HOURS

Our office is open from 7:45 AM to 4:45 PM Monday through Friday.

FEES

Fees charged are based on the time and skill required for each procedure. We request payment at the time the service is rendered.

 INSURANCE

We are a participating provider for numerous insurance companies.  As a service to you we will file your insurance for you.  You will be responsible for routine examinations, deductibles not met, and co-insurance percentage not paid on the approved amount.  It is the patient’s responsibility to verify with their insurance that we are a participating provider prior to your visit. Please be sure to provide us with complete insurance information and personal identification (such as driver’s license) when you check in with the receptionist.

 EMERGENCY AFTER HOURS

If you have an emergency after hours or on weekends, call our office at 478-741-1740 and the on-call doctor will be paged.

 TREATING MINORS

We limit our practice to patients 18 and above.

MEDICAL RECORDS

Your medical records will not be released by us without your written consent.  A minimal fee is charged when records are duplicated.
